Forms
CHDI.org has Join Email List and Contact Us forms users may choose to fill out and submit on the website. These forms are completely voluntary and you may choose whether or not to disclose personal information, such as name, Company, and comments. Email addresses collected through the Join Email List form will be used to send emails from CHDI, including CHDI’s newsletter, publications, and relevant resources and updates on CHDI and children’s behavioral health issues. Email addresses and other personal information collected by CHDI and CHDI.org will not be sold or shared with third parties.
CHDI.org website users can choose to click on the Donate button to donate CHDI through PayPal (view PayPal policies). Information collected by PayPal and shared with CHDI from PayPal such as name, address, and amount of donation will not be used, shared, or disclosed for purposes other than to fulfill the donation request.
CHDI.org requires users interested in downloading the Child Trauma Screen (CTS) to complete a survey asking how they intend to use the CTS, how they heard of the CTS, and personal information such as their name, organization, title/role, email, country, and state. Information collected through the CTS survey may be used by CHDI for research and to inform survey participants about updates to the CTS or share relevant resources about childhood trauma. Information collected will not be sold or shared with third parties.

We take precautions to protect your information. When you submit sensitive information via the website, your information is protected both online and offline.
