PROMOTING BETTER COORDINATION OF CARE

CHDI has long recognized the need for better care coordination across child-serving systems.

Too often care can be fragmented, with several entities coordinating children’s care without consistent communication or integration. As a result, children and families often are not linked to available resources and organizations are not aware of the services that others provide.

Hartford Care Coordination Collaborative (HCCC)

CHDI supported the 2010 development of the Hartford Care Coordination Collaborative (HCCC) to promote care coordination across health, mental health, early care and education, child welfare, dental care, family support and other community services.

The HCCC brings together several entities that coordinate care for children in the greater Hartford region, including:

• Community Health Network
• Behavioral Health Partnership staff
• Dental Health Partnership staff
• CT Family Support Network
• Department of Children and Families

The HCCC model:

• Reduces duplication of services by facilitating communication across sectors
• Provides effective and efficient linkage to services for children and families
• Fosters a shared mission for care coordination in the region

A Statewide System of Care Coordination

The successful HCCC model was replicated statewide. In 2014, the State Department of Public Health allocated funding to five regional care coordination centers. These centers work with the Office for Community Child Health at Connecticut Children’s and CHDI to develop care coordination collaboratives.

This statewide system of care coordination:

• Serves families throughout Connecticut through five regional hubs
• Supports pediatric primary care sites as they transform their practices into Person-Centered Medical Homes.
