CONNECT4Families Toolkit

The Connect4Families Toolkit helps families, schools, and pediatric providers collaborate and connect children to behavioral health services and supports in Connecticut. 

The toolkit includes a comprehensive service directory to facilitate referrals, along with best practices, tips, strategies, customizable communication forms, and more. 

 

The Connect4Families Toolkit was produced by the Connecting to Care initiative’s Family Care Connections (FCC) Workgroup, nationally recognized integrated care expert Barbara Ward-Zimmerman, Ph.D., the Child Health and Development Institute (CHDI), Carelon Behavioral Health, The Consultation Center at Yale, and FAVOR, Inc. 

CHDI serves as the Coordinating Center for the Connecting to Care Initiative.
